Service provides assessment, diagnosis, treatment, and education for people who have a thickened heart muscle (hypertrophic cardiomyopathy).
The Hypertrophic Cardiomyopathy Clinic (HCM) is a Nurse Clinician directed clinic with multidisciplinary support dedicated to the management of patients with suspected or confirmed HCM. Plan of care includes interval clinical assessment and follow-up by a cardiologist. Services provided may include:
- assessment, diagnosis, treatment, and education for people who have hypertrophic cardiomyopathy
- management of prescription medications
- ongoing patient teaching
- lifestyle recommendations
- family screening teaching
- coordination of necessary specialist referrals
