This central access referral service provides a single point of entry for patients who want to access Children’s Rehabilitation Services in the Central Zone.
The services provided support the development of children and youth to participate in meaningful activities of daily life at home and in the community. Services are goal-driven; starting and continuing services is based on having functional reasons / goals for rehabilitation.
Therapists will work with parents to provide the tools and strategies to apply in the context of their home and life. We will work together so you and your child are supported to move to the next developmental stage or between providers and services.
Social Work and Registered Dietitian services are available to support rehabilitation outcomes.
Our social workers will connect the child with the appropriate Therapists on the Children’s Rehabilitation Services Team including Speech Language Pathologists, Physical Therapists, Occupational Therapists and other providers. The Social Worker may refer the client to other community based services or other health providers as needed.
