Provides outpatient ongoing follow-up care, management, and education for patients living with heart failure.
Services this program provides include:
- initial and ongoing assessment of condition
- providing individualized patient education to equip self-management of heart failure
- optimization of heart failure care including medication use and dose adjustments
- patient / family support
- ordering appropriate diagnostics / interventions as necessary
- referrals to specialists and other programs
