Support For Persons With Disabilities
Offers 1-on-1 support to individuals with developmental disabilities, mental health disabilities, and / or addictions to expand their social networks, develop social skills, and improve positive interaction.
Service provides:
- 1-on-1 support to individuals with developmental disabilities, mental health disabilities, and / or addictions to expand their social networks, develop social skills, and improve positive interaction
- person-centred planning and individualized goal setting, through which social skills are further developed while the individual works toward enhancing positive relationships, improving their sense of well-being, and inclusion in the community
Includes:
- community living supports and accessibility
- employment and educational support; assistance acquiring or maintaining employment, resumé preparation, application, and funding inquiries for school
- addressing physical and health needs
- support with maintaining housing
- support with justice
- support for enhancing cultural values and beliefs
- personal accountability
- support for complex needs
