Offers support to individuals 50 years and older to identify and access available community resources.
Service provides:
- drop-in services and home visits
Drop-in services (office) include:
- 1-on-1 confidential information
- holistic assessment of needs
- referrals
- assistance in understanding and filling out government forms
- follow-up with government officials
- linking to food security programs
- advocacy
- linking to transportation services
- and basic education and advice on how to address elder abuse
Home visits by the senior systems navigator (social worker) include:
- assessing needs
- making subsequent referrals to community-based services and supports
- helping clients connect with government officials
- and providing case updates to participants with mobility challenges who can't leave their home
